In order to be in compliance with Florida Department of Health State Laws,
all patients must have their own Patient Profile with accurate
First Name, Last Name, Birthday, Phone Number, Email and a valid Credit Card on file.

Every scheduled appointment must be coordinated by the patient themselves as responsibility
of their own medical record. We do not allow booking for family members/friends.
Appointments are non-transferable.

DEPOSITS
A $100 non-refundable deposit is required for each individual service on the schedule
at the time of scheduling in order to secure your appointment time with your provider.
RESCHEDULED/CANCELLED APPOINTMENTS
All appointments must be rescheduled at least 24 hours in advance.
In order to avoid losing your $100 deposit(s), appointments must be rescheduled and NOT cancelled.
This is the only way to have your $100 non-refundable deposit transferred to your rescheduled appointment.
We will only allow a max of 3 reschedules for any set appointment.

TOX TOUCH UP POLICY
Initial Treatment Timeline
Due to the nature of neurotoxins, full results may take up to 14 days to manifest. Please be patient as your treatment takes effect.
Touch-Up Policy
Each tox treatment includes a single touch-up session, if needed, to ensure optimal results. The timing for touch-ups is determined by your provider and typically falls within 2–3 weeks following the initial treatment.
Scheduling Requirements
You must schedule this appointment on the day of your initial tox treatment. Scheduling the touch-up at this time ensures availability within the necessary window.
No Touch-Ups After 3 Weeks
Due to the time-sensitive nature of neurotoxins, touch-ups cannot be scheduled/rescheduled or performed after the 3-week mark. We appreciate your understanding in adhering to this policy.
Always use the Patient Portal as a first attempt to cancel or reschedule appointments.

If attempting to cancel over the phone, please leave a detailed voice message
with your First and Last name and date of appointment, as well as sending a text to our text line for further communication.

We apologize in advance for not always being able to attend to the phone.
Patients in-office are always our #1 priority. We ask that when calling and there is no answer, please leave a voicemail so that we may return your call. All emergency messages are answered first.
For all other inquiries, please allow one full business day (M - F, 9:30 am - 4:00 pm) for a response. If your call is in reference to an appointment, please keep in mind that you have access to your patient portal 24/7 and have the ability to cancel/reschedule any upcoming appointments through your portal, as well as book new appointments, fill out patient forms, sign consents, etc.

NO SHOW APPOINTMENT PENALTY
All appointments must be cancelled/rescheduled at least 24 hours in advance to the appointment time to avoid an additional $75 fee on top of your $100 non-refundable booking deposit.

CONFIRMATIONS
Confirmations are sent by both email and text 3 days prior as well as 26 hours prior to your appointment. It is the patient's responsibility to monitor and reply to these attempts to avoid any additional charges. Any late responses will not be accepted.

LATE ARRIVAL
A grace period of 5 mins will be permitted when you are scheduled for a Tox appointment (15 min appt. duration), and a grace period of 10 mins when you are scheduled for a Filler appointment (30 min appt. duration) will be permitted. Anything over these grace periods, you have officially missed your appointment and a NO SHOW fee of $75 (non-refundable) will be charged. If you arrive late, you may still be seen, depending on the provider’s availability. However, not all services may be permitted due to time and your late fee of $75 will not be applied towards your balance. If you arrive after your appointment time has ended and provider is not able to see you due to tardiness, you will NOT be seen and any deposit/s attached to that appointment will be lost.

SCHEDULING FUTURE APPOINTMENTS
All patients are permitted to have multiple upcoming appointment on the schedule at any given time.
Deposits are required for each individual service on the schedule.
Provider schedules are unlocked 1 quarter (3 months) in advance.​​

NON SURGICAL NOSE JOB - effective January 1st 2023

Beginning January 1st 2023 there will be a PRICE INCREASE to all Non Surgical Nose Job treatments.
The cost to all Non Surgical Nose Job procedures will now be $1200, whether it will be for a first time treatment or
for maintenance treatments.​
PARKING UPDATES
Please park in the complimentary parking lot adjacent to our building or across the street in the open lot in any of the UNRESERVED spaces (for customers of 5901 building only). If you have any plans before or after your appointment please park in their designated area or your car will be towed. Selfie Aesthetic is not responsible for the towing or ticketing or any vehicles. Please allow a couple extra minutes for parking.
